How appointment requests work

  • Submit the brief Request Appointment form with contact and administrative information.
  • The practice reviews your request and contacts you by your preferred method about availability and next steps.
  • If the practice accepts your request, follow the instructions provided to complete secure patient setup through TherapyPortal.
  • Join your scheduled telehealth appointment at the confirmed time.

Submitting a request does not confirm an appointment. The public form is for initial contact only; keep clinical information in the secure intake workflow provided by the practice.

Cancellation & Rescheduling
